New Patients

To ensure a smooth visit, please bring your insurance information/card, photo identification, and any relevant dental/medical records with you.

Prior to your appointment, download, print, and complete the appropriate patient information form. If you are unable to do this, please plan to arrive 15 minutes early so you can complete this before your appointment time.

We accept most dental insurance plans. If you have questions about your coverage, contact your insurance provider. You are responsible for any services that are not covered, including co-pay, co-insurance, etc.

Our office accepts cash, Visa, Discover, MasterCard, Care Credit, and City Health for payment.
